Schervier will receive grant for Transitional Care Program

The Schervier Nursing Care Center has received a $103,000 grant from the Fan Fox & Leslie R. Samuels Foundation to fund a Transitional Care Program, which will aid patients moving from the hospital to their Riverdale facility.

The program aims to reduce the cost of care and prevent the need for patients to be re-admitted to the hospital.

A social worker will be hired as part of the program to act as a transition coach. The transition coach will help support patients and their family members to ensure that their wishes and needs are known to the medical staff. The coach works closely with the staff and patients throughout the transition.

“We are thankful to the Fan Fox & Leslie R. Samuels Foundation for its generous donation and are anxious to implement this new position as we continue our mission of providing good help to those in need, especially those who are poor and dying,” said Schervier CEO Eileen Malo in a press release.
